Health Promotion Study Visit 3 (Portugal)
A study visit in Lisbon took place on 23rd-24th May. Health Promotion Study Visit 1&2 (the Netherlands)
RIVM organised a study visit on their selected good practice 'JOGG', the Young People at a Healthy Weight project, which encourages all people in a city, town or neighbourhood to make healthy food and exercise an easy and attractive lifestyle option for young people, as well as a second study visit to introduce their good practice database to interested partners. Multimorbidity Expert Meeting
The main achievement during 2015 for the JA-CHRODIS work package on multimorbidity was the development of a framework for care of multimorbidity patients that can be applied across Europe. First, we identified a list of 20 components across five domains from existing published comprehensive care programs for patients with multiple chronic conditions or frailty.
