The study visit in Zagreb presented the work of Croatian Institute of Public Health in the field of diabetes registries.
Well established electronic patient registries improve quality indicators of diabetes control at population level due to better insights in patients’ health status, improved health care delivery and better patient complience as well as continuous organisational improvement within and between involved health care providers. Specific aim of using the tool of Quality Criteria Recommenations in this implementation project is to increase use of diabetes control check-list as a minimum data set within diabetes registry.

13.45 – 14.15 LIWG presents assesment of the Pilot Action implementation with respect to the criteria on scalability and sustainability, building also on the result of first part of the study visit day
14.15 – 16.15 Debate with other participants from JA CHRODIS+ with an aim to identify actions that would increase scalability and sustainability of the practice
