JA-CHRODIS work package on multimorbidity organises a conference on how to achieve better prevention, management and care of chronic diseases for a healthy and active ageing, which will take place in Rome on the 2nd of February.

The one-day event will give an overview of the Joint Action results and of how they can used to promote healthy and active ageing in Europe. Presentations will focus on the work done in the fields of diabetes, health promotion and multimorbidity, and the patients’ perspective will also be represented.
