Multimorbidity Expert Meeting
The main achievement during 2015 for the JA-CHRODIS work package on multimorbidity was the development of a framework for care of multimorbidity patients that can be applied across Europe. First, we identified a list of 20 components across five domains from existing published comprehensive care programs for patients with multiple chronic conditions or frailty. 18th European Health Forum Gastein
JA-CHRODIS was present at the 18th European Health Forum Gastein, a three-day event held on 30 September to 2 October 2015 in Austria. The Forum is one of the most important health-related event in the European Union and a meeting point for experts from different fields within the health system. On 10-11 September 2015, JA-CHRODIS will present on multimorbidity in Lithuania at the Conference of Baltic Family Doctors in Pärnu, Estonia. The conference represents an excellent opportunity to share news and exchange personal experiences from the field of family medicine, as well to provide valuable information and insights for family doctors and nurses. 2nd WP6 Partners Meeting
On the 10th June 2015, the JA-CHRODIS partners working on multimorbidity held their second partners’ meeting in Treviso, Italy. The meeting provided partners with an opportunity to review the activities performed during the first year of JA-CHRODIS, to assess the ongoing works and to plan the next steps for future action. Joint WP6 and WP7 Partners’ Meeting
On the 6th and 7th November 2014, WORK PACKAGE 6 (MULTIMORBIDITY) and WORK PACKAGE 7 (TYPE 2 DIABETES) met in Vilnius, Lithuania.
