SUMMARY:Visit to Spanish implementation site in Zaragoza
DESCRIPTION:On 21th June 2019\, Carmen Rodriguez-Blazquez and Maria João Forjaz (ISCIII) held a visit to Aragon Health Sciences Institute (IACS)\, in Zaragoza\, Spain. The primary care centre Actur-Norte and the Chronic Care Unit at Royo Villanova Hospital were both visited. The implementation team presented the three main strategies of the practice: training of professionals in multimorbidity (eMultiPAP)\, integrating complex chronic patients components (comprehensive assessment\, virtual interconsultation\, and electronic health records)\, and community care actions (health needs identification and resources map). The meeting closed with a debate on the facilitators\, barriers and benefits of the program.  \n \n  \n
