SUMMARY:Visit to Spanish implementation site in Seville
DESCRIPTION:On June 7th 2019\, Carmen Rodriguez-Blazquez (ISCIII) and Elisa Poses-Ferrer (AQuAS) visited San Jose de la Rinconada primary care center in Seville. Visitors received information on the Andalusian Comprehensive Healthcare Plan for Patients with Chronic Diseases (which includes training activities to health professionals)\, and the way Individualized care plans for multimorbid patients are completed within the corporate electronic health record (Diraya)\, and shared with patients interviewed by healthcare professionals. Participants also had the opportunity to discuss the benefits\, experienced barriers and facilitators of the pilot action with the local implementers. \n \n
