DESCRIPTION:JA-CHRODIS work package on multimorbidity organises a conference on how to achieve better prevention\, management and care of chronic diseases for a healthy and active ageing\, which will take place in Rome on the 2nd of February. \nThe one-day event will give an overview of the Joint Action results and of how they can used to promote healthy and active ageing in Europe.
