Beyond the inherent difficulties related to the construction of a platform for knowledge exchange, we are fascinated by the challenge of building an agora where decision-makers, caregivers, patients, and researchers, can be able to exchange the best knowledge on chronic care across Europe.
Andalusia, the largest and most populated region in Spain, has a long-lasting interest and experience in the field of health promotion and prevention. Through contributing to JA CHRODIS, we hope to exchange and enhance the know-how we all hold in this area, for the public health improvement and the general benefit.

Multimorbidity is very common for the Basque population and its prevalence rises in age and unfavorable socioeconomic environment. Given the ageing population, multimorbidity and its consequences should be taken into account in health policy, the organisation of care and medical research. We believe that JA CHRODIS will provide us deeper knowledge on this topic.
Galician Healthcare Service
Diabetes is the paradigm of chronic diseases, with important consequences for both the patient who suffers it and the healthcare system which supports their. It appears in fourth place in the Galicia Priorites Plan, after heart disease, lung cancer and stroke because of the prevalence of this disease in our region. Contributing to JA-CHRODIS is a great opportunity to share and learn more about the best way to manage this disease.
